Terms of Use

Terms of Use

Bank will issue gift cards in the amount designated by Payor. Funding for the gift card must be provided by Payor at the time of issuance. You must activate your card by calling 800.710.4795 and selecting your PIN. The gift card cannot be used until it has been activated. Gift card purchase is only available to MidFirst customers. Payor agrees to pay $3.75 for each gift card purchased. Gift cards may not be exchanged for cash. A $5 monthly fee will be assessed after 12 consecutive months of inactivity.

Use of Gift Cards

Gift card may be used by Customer at any retail outlet that accepts Visa® or MasterCard® branded cards. Use your gift card to make a purchase exactly like a credit card. You must select a PIN to activate your gift card; however your gift card does not require a PIN to complete a transaction. By accepting and using your gift card, you are agreeing to the terms set forth in the gift card terms of use. Gift card will only be accepted for purchases up to the available balance on gift card by the amount of the transaction. When all of the remaining funds have been depleted from gift card, the gift card will no longer be usable. Gift card is not re-loadable, which means no additional funds may be added to the remaining balance of the gift card. Gift card may not be used to withdraw cash from an ATM, to transfer balances from one card to another or to obtain cash advances.

Balance and Transactional Inquiries 

Customer will be able to access balance and transaction history information by visiting online gift card access, or by calling 800.710.4795.

Lost or Stolen Gift Cards

In the event that gift card is lost or stolen, Customer agrees to contact Bank as soon as possible at 800.710.4795. Customer must provide the gift card number. When Customer alerts Bank of a lost or stolen gift card, Bank will cancel the gift card. Customer may request the issuance of a new gift card. If Bank issues a new gift card, Customer will be charged a ten dollar ($10.00) fee which will be deducted from the remaining balance on the gift card.

Dispute Resolution

If Customer believes any transaction or receipt is incorrect, Customer must notify Bank no later than sixty (60) days after the date of the transaction. Customer may notify the bank by calling 800.710.4795.
